Helping children reach the care they need
Healthy development is shaped by access to appropriate care, nutrition, information and supportive family circumstances. ActionReach helps identify children who have unmet health or nutrition needs and works with caregivers and responsible providers to address barriers that prevent agreed care from being accessed or completed.

Areas of support
- Helping families navigate health-facility access and follow-up appointments.
- Supporting completion of agreed care pathways where a child has been identified as needing follow-up.
- Connecting caregivers with appropriate nutrition services and guidance delivered by qualified providers.
- Helping families understand the next steps after a referral or clinical visit.
- Identifying practical barriers that prevent attendance or continuity of care.
- Following up to establish whether the agreed care was received, while respecting clinical confidentiality.
Outcome orientation
ActionReach measures progress by whether the agreed health or nutrition action was completed and appropriately verified, not simply by whether information was shared or a referral issued. Clinical decisions remain with qualified health professionals.
2026–2031 target: 16,000 children completing agreed health or nutrition care by 30 June 2031. This is a target, not an achievement.
